Scattered Site Programs
Urban Pathways Scattered Site Programs
Scattered site living provides clients with special needs an opportunity to live in the community in an apartment
setting while still receiving supportive services. These programs assist individuals with their reintegration
into the community and move toward greater stability and independence. Clients work with Case Managers to
develop mutually agreeable goals and service plans aimed at improving their individual lives. Some of the
services provided include:
- Daily Living Skills and Money Management
- Positive Social Networking and Family Integration
- Medication Management
- Vocational and Educational Services
- Health and Medical Services
- Crisis Intervention
- Client Self Advocacy
Urban Pathways currently operates three such programs:
Apartment Treatment Program
The Apartment Treatment Program (OMH Funded) is the program's newest addition starting operation in January
of 2011. ATP is a 40-bed, 20 unit, OMH licensed transitional supportive housing program. The program
consists of two residents sharing a two bedroom apartment. All units in the ATP program are located in the South
section of the Bronx, NY. The goal for residents in this program is to develop and/or enhance independent livings
skills that will enable and ensure a successful transition to less restrictive housing. In order to apply for
housing, candidates must have the following:
- Active NY/NY HRA 2010E housing application with Level II approval
- SSI Benefits
- Active Medicaid
- Willing to share a two bedroom apartment with one other resident
DOHMH Scattered Site
The DOHMH Scattered Site (DOHMH funded) program is intended for individuals who have or have a history of
substance abuse. It is a 26 unit DOHMH funded supportive housing program. Services are provided using
a Harm Reduction/Trauma Sensitivity Approach that utilizes wellness self management model. The goal for residents
in this program is to identify and address substance abuse issues and seek treatment which will assist resident with their
rehabilitation endeavor. In order to apply for housing, candidates must have the following:
- Active NY/NY HRA 2010E housing application with Level III, Category "E" approval
- Active entitlements (SSI, SSD, PA benefits)
- Able to live within a minimal supportive environment
OMH Scattered Site
The OMH Scattered Site (OMH funded) program is designed for individuals who are diagnosed with a mental
illness. It is a 41 unit OMH funded supportive housing program which includes 7 Manhattan units dedicated to
OMH Psychiatric Center long term stayers. Primary goal for residents in this program include obtaining and
maintaining psychiatric stability through participation in community linked programs. We currently maintain
apartments in the Bronx and Manhattan. In order to apply for housing, candidates must have the following:
- Active NY/NY HRA 2010E housing application with Level III, Category "A" approval
- Active entitlements (SSI, SSD, PA benefits)
- Able to live within a minimal supportive environment
