Funded by The Flatiron/23rd Street Partnership BID, these teams conduct outreach from 21st to 28th Street and from Third
Avenue to Sixth Avenue. This district tends to contain some of the most difficult, isolated chronic homeless
individuals in the City; transient in nature, they often move between this BID's catchment area and Penn Station where our
Amtrak Outreach Program is housed. We continually engage this population, offering referrals
to drop-in centers, shelters, safe havens, detox/rehab facilities, and medical facilities. Clients with an extensive history
of living on the streets are referred to a Manhattan Outreach Consortium team to be placed into
permanent housing.

Placed 10 clients in hospitals for medical or psychiatric treatment
