Outreach Programs


According to the New York City 2010 HOPE Count there are an estimated 3,111 unsheltered individuals living on the streets or in places unsuitable for human habitation.   Urban Pathways' outreach programs work with this population, helping them to leave the streets, become housing-ready through a rehabilitative process and move along the path to self-sufficiency and permanent housing.

Our outreach programs are staffed by teams that creatively address the needs of difficult to reach populations through consistent low-threshold engagement, complemented by individualized assessment, intensive one-on-one case management and strong ties to a wide network of referral services.  These programs engage consumers ‘where they are at’ and engage homeless adults through outreach, case management and referrals in order to move them into housing and treatment and minimize the chance of a return to the streets.

Many of the homeless are first introduced to services through outreach teams.  It is typical for the outreach worker to be the first person who makes contact with a homeless individual.  The outreach worker will perform an assessment of the client and make the appropriate referral to a shelter or treatment facility based on that assessment.  In many cases, the outreach team will escort the client to the program.

Urban Pathways’ outreach teams place a high priority on targeting those that are chronically homeless in an intense and systematic manner.  Teams work on dual fronts. While conducting street outreach, they work on obtaining long-term housing to those that have been or are known to be homeless by assessing their duration of homelessness and determining whether their needs require long-term intervention or can be resolved by means of immediate referral and traveler’s assistance.  For those individuals that are self-referred, referred by police or management, or seeking immediate services, staff will conduct preliminary assessments, crisis intervention, and arrange referrals to a wide spectrum of service providers with whom Urban Pathways has long-standing linkages.  We place clients into permanent housing, transitional housing, detox/rehab programs, shelters, drop-in centers, and medical facilities.
